  • 1. Prevention & awareness promotion

    We aim to help citizens to better understand dementia and people with dementia, through various education and public relation programs, and to have them actively participate in various programs of the SDMP. This project is expected to not only promote activities of citizens for dementia prevention and management but also make dementia-friendly communities in Seoul. The ¡°Ten Million Citizen Memory Friends¡± is one of the representative programs developed to improve the public awareness of dementia.
  • 2. Early detection

    By applying systematic screening and examination to community-dowelling citizens in Seoul, we aim to detect people with dementia or those at high-risk state as early as possible, in order to prevent or delay the progression of cognitive deterioration. The activities include general counselling, screening with a brief screening tool, diagnostic examinations for screened cases, and referral to clinic for further laboratory work up for possible dementia cases.
  • 3. Case registration & management

    This is a project that (1) registers the cognitively impaired (with dementia or at high risk state) or cognitively normal individuals classified by the early detection project and (2) provides appropriate management services according to the diagnostic status. For people with dementia and their family, customized management services tailored to the condition are provided to delay deterioration, improve quality of life, and reduce family burden. For the individuals at high-risk state or cognitively normal individuals, various programs including education, cognitive promotion programs and regular check-up are provided to help dementia prevention.
  • 4. Networking and strengthening of community-resources

    This project aims to establish well-functioning dementia service networks (DSNs) in a community. To achieve this goal, we strive to strengthen existing resources in the community for dementia management, such as welfare center for the elderly, day-care centers, nursing homes, and clinics of primary care physicians. Furthermore, we support to strengthen the networks among them or with local dementia centers, through education and training programs, technical supports, and various types of meetings.
  • 5. Online-based support & monitoring

    The online-based support and monitoring is put into place to efficiently provide information about dementia and the SDMP to citizens though webpages or smart phone. It also functions to execute and monitor the progress of the SDMP through the online database system. The online database system also helps us to summarize the progress of each component of the SDMP quickly, which is used for future plans, feedbacks and adjustments of the ongoing projects.
